    I have to agree with shendog. Your best bet would look in the yellow pages under "cement contractors" and from there look for cement contractors that do industrial, slab & road work.
    5/8" #5 is some very heavy stock and only used in commercial work.
    You may try some scrap yards also, talk to the boss! and tell him/her what you need and what for.
